We will not be able to register patients on 02.12.2015 as we are changing computer systems from Systmone to EMIS WEB. We will hold the registrations until the 03.12.2015 and add them as soon as possible on that day. Thank you for your patience and we are sorry for any inconvenience this may cause.
WE HAVE HAD TO CHANGE HOW PATIENTS REQUEST REPEAT MEDICATIONS and patients need to use a different website and password to do so. If you previously requested medications via this website please call in to the surgery with a form of photographic ID either passport or drivers licence card or a document which verifies your address. The receptionists will give you a new user name and password to enable you to register for online access
PLEASE REMEMBER TO INFORM THE SURGERY IF ANY OF YOUR DETAILS CHANGE EG ADDRESS HOME TELEPHONE OR MOBILE TELEPHONE NUMBER
WE ARE SORRY TO INFORM OUR PATIENTS THAT the sURGERY IS NO LONGER OPEN ON SATURDAY MORNINGS.
How information about you helps us to provide better care
Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone. This information along with your postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information. This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.
You have a choice . If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please speak to practice staff or ask at reception for a copy of the leaflet “How information about you helps us to provide better care”.
